Valid Driver’s License: Clear, high-resolution photos of the front and back of your active driving license.
Vehicle Photos: High-quality images of your vehicle from multiple angles. Note: The license plate must be clearly visible and legible in the photos.
Proof of Insurance: Copy of your active insurance document (Personal or Commercial liability policies are accepted).
Motor Vehicle Record (MVR): A clean driving record printout (MVR) from the last 3 months is highly preferred.
Airport Permits (If Applicable): Valid PHX Sky Harbor or regional airport commercial vehicle permits if you already operate them. ( Placard )
Company Details: If you operate under an LLC or registered business entity, please provide your legal Company Name.
Accepted Formats: Only JPG, JPEG, PNG, doc and PDF files are permitted.
